Insurance

We gladly submit insurance claims to any dental plan unless specific providers are designated on your plan. We will be happy to file your insurance claims for you, as long as you are free to choose your own dentist. We are a Delta Dental Premier Provider and are In-Network for Anthem BCBS – Federal Plan only. Please bring all information you have regarding your insurance, such as mailing address and policy and group numbers.  It will benefit you to be aware of your benefits. Despite insurance coverage, you are ultimately responsible for all charges.  In an effort to help you understand more about insurance benefits, we’d like to share some facts about dental insurance with you…

  • Your dental insurance is based on a contract between your employer and the insurance company.  Every plan will differ, even if it is the same insurance company, based on what the employer has contracted.  In an effort to save money, we see plan benefits changing all the time.  While we try to stay on top of these changes it is impossible for us to do as we are not notified when changes take place.  It is important for you to try to stay updated, and to keep us updated.  Should questions arise about your coverage, it is best for you to contact your human resources department or the insurance company directly.
  • You may receive notification from your insurance company stating dental fees are “higher than usual and customary”. An insurance company surveys a geographic area, calculates an average fee and then takes that fee and considers it customary.  Included in this survey are discount clinics and managed care facilities, which bring down the average.  Most doctors in private practice will have fees, which are defined as “above usual and customary”.
  • Many plans tell participants they will be covered up to 80% or up to 100%, but do not clearly specify plan fee schedule allowances, annual maximums, waiting periods or limitations.  Insurance companies do NOT cover all dental services.

Help Us Help You

Our practice is dedicated to quality care and exceptional service.  We respect the importance of your time, and work very hard to schedule appointments that accommodate the busy scheduling needs of all out patients.  In return, we ask that patients make every effort not to change reserved dental appointments.  Failed appointments create scheduling problems for other patients as well as the practice.  If you find that you must change your appointment, we ask 24 hour notice so that we may accommodate another patient.  We reserve the right to charge a fee for any broken or failed appointment.  Thank you for your cooperation in this matter.
